Acne Scars Treatment: What Actually Works by Scar Type

Ice-pick, rolling, and boxcar scars each need different tools. Here's how an acne scars treatment plan is matched to your skin— microneedling, laser, fillers, and more.

Acne Scar Treatments: What Actually Works for Each Scar Type

If you've spent years covering up textured skin, you already know that acne scars treatment isn't one-size-fits-all. Ice-pick, rolling, and boxcar scars each respond to different tools — microneedling, energy-based resurfacing, subcision, and dermal fillers — and choosing the right combination is what separates a modest improvement from a genuinely smoother complexion. At MedexClinic's medi-spa in Jacksonville, FL, our team builds layered plans that target the specific scar shapes on your face, not a generic protocol.

This guide walks through how to identify each scar type, which in-office treatments tend to work for each, what a realistic timeline looks like, and how to get started with a personalized consultation.

Why acne scars form in the first place

Acne scars are the aftermath of inflammation. When a deep pimple or cyst damages the dermis, the skin repairs itself with collagen — but that repair is rarely perfect. If too little collagen is produced, the skin sinks in and you get an atrophic scar (ice-pick, rolling, or boxcar). If too much collagen is produced, you get a hypertrophic or keloid scar that sits raised above the skin. Discoloration that lingers after a breakout — red, brown, or purple — is called post-inflammatory hyperpigmentation (PIH) and technically isn't a scar at all, though it's often lumped in.

Identifying which of these you're dealing with is step one, because the treatments differ dramatically.

How do I know what type of acne scar I have?

Most people have a mix of scar types, but here's how clinicians typically categorize them:

  • Ice-pick scars — narrow, deep, V-shaped pits that look like the skin was punctured with a sharp tool. Often on the cheeks and temples.
  • Rolling scars — broad, shallow depressions with sloping edges that give the skin a wavy, undulating appearance. Tethered from below by fibrous bands.
  • Boxcar scars — round or oval depressions with sharp, well-defined vertical edges, like a chickenpox scar. Can be shallow or deep.
  • Hypertrophic / keloid scars — raised, firm tissue, more common on the jawline, chest, back, and shoulders.
  • Post-inflammatory hyperpigmentation — flat brown or red marks left after a breakout heals. These often fade on their own but can be accelerated with treatment.

A quick way to self-check at home: run a fingertip over the area. If you feel a distinct edge, it's likely a boxcar. If it feels like a soft dip, it's rolling. If it feels like a pinpoint pit, it's ice-pick.

Matching the treatment to the scar

Below is a general framework for which medi-spa treatments tend to work for which scar shape. Your actual plan should be built with an experienced provider after an in-person skin assessment.

Microneedling — for rolling scars and overall texture

Microneedling uses a device with fine, sterile needles to create controlled micro-injuries in the dermis. Those tiny channels trigger the skin's wound-healing cascade — new collagen and elastin fill in the depressed areas gradually over several months. It's a workhorse treatment for rolling scars and for improving general skin tone and pore size, and it also helps soften shallow boxcar edges. Most patients need a series of sessions spaced a few weeks apart. Downtime is usually a day or two of redness that feels like a mild sunburn.

Laser and energy-based resurfacing — for boxcar scars and pigmentation

Fractional and IPL (intense pulsed light) treatments deliver targeted energy that resurfaces the top layers of skin and stimulates collagen deeper down. IPL photofacials are particularly useful for red and brown post-acne pigmentation, while fractional resurfacing helps sharpen and soften the vertical walls of boxcar scars. Downtime varies with the intensity chosen — from mild pinkness to a week of peeling — which is why your provider will match the setting to your skin type and schedule.

Chemical peels — for shallow scars and pigmentation

Medi-spa chemical peels use acids (glycolic, salicylic, TCA, or blended formulas) to exfoliate the outer skin and encourage even regeneration. Lighter peels are well suited to PIH and dull texture; medium-depth peels can soften shallow boxcar and rolling scars. Peels layer beautifully with microneedling in a longer-term plan.

TCA CROSS — for ice-pick scars

Ice-pick scars are notoriously stubborn because they're too narrow and deep for microneedling or general resurfacing to reach the bottom. TCA CROSS (Chemical Reconstruction of Skin Scars) is a technique where a tiny drop of high-strength acid is placed directly into the pit, prompting collagen to fill it in from the base upward. Multiple treatments are typically needed, but for the right candidate the results can be meaningful.

Subcision and dermal fillers — for tethered rolling scars

Some rolling scars are pulled down by fibrous bands under the skin. Subcision uses a fine needle to release those tethers, letting the skin lift back up. A dermal filler can then be placed in the depression to support the correction while natural collagen rebuilds. Fillers can also camouflage isolated deeper scars while a longer-term collagen-building plan does its work.

What does a realistic acne scar plan look like?

Because most people have a mix of scar types, layered plans consistently outperform any single treatment. A typical sequence at our medi-spa might look like:

  • A skin consultation to map scar types, skin tone, and any active breakouts.
  • Getting active acne under control first — treating scars while breakouts continue creates new scars.
  • A course of microneedling and/or IPL to address rolling scars, texture, and pigmentation.
  • Targeted treatments (TCA CROSS for ice-pick, subcision or filler for tethered rolling) layered in.
  • A daily home routine — sunscreen, gentle retinoids as tolerated, and barrier care — to protect and extend results.

Patience matters. Collagen remodeling is a months-long biological process. Real, visible improvement typically shows up gradually over three to six months and continues past the final in-office session.

Every skin plan at MedexClinic is doctor-supervised, so treatment intensity, sequencing, and any prescription skincare are calibrated to your skin type and health history rather than a generic template. If a medication ever becomes part of a broader plan, it's always framed as one component of a supervised approach — never as a shelf product to buy.

How to prepare for your first acne scar consultation

  • Come with a clean face if possible — makeup makes scar mapping harder.
  • Bring a list of skincare products you currently use, especially retinoids, exfoliating acids, or hydroquinone.
  • Note any history of cold sores, keloids, or delayed wound healing.
  • Tell your provider about any recent sun exposure or upcoming events with downtime constraints.
  • Have questions ready about expected timeline, aftercare, and which treatments will be layered.
