Anti-Wrinkle Injections: A Doctor's Guide for First-Timers

If you're curious about anti-wrinkle injections but you've never sat in a treatment chair before, you're in good company. Wrinkle-relaxing injectables are one of the most requested cosmetic treatments at our medi-spa, and most first-timers arrive with the same questions: Will it hurt? Will I look frozen? How long does it last? This doctor-written guide walks you through what these injectables actually do, where they're used, and what to expect from your consultation through the two-week mark — so you can decide whether treatment is right for you.

What are anti-wrinkle injections?

Anti-wrinkle injections are a category of wrinkle-relaxing injectables — small, precisely placed doses of a purified protein that temporarily reduces the contraction of targeted facial muscles. When those muscles relax, the overlying skin smooths, and the dynamic lines that appear when you frown, squint, or raise your brows soften.

They are not the same as dermal fillers. Fillers add volume beneath the skin to restore lost fullness or soften static folds. Wrinkle-relaxing injectables work on the muscle layer to prevent repeated creasing. Many patients ultimately benefit from a thoughtful combination, but they solve different problems.

Which areas are most commonly treated?

Three areas account for the majority of first-time treatments, because they're where expressive muscle movement creates the earliest visible lines:

  • Forehead lines — the horizontal lines that appear when you raise your eyebrows.
  • Frown lines (glabellar "11s") — the vertical lines between the brows that deepen when you concentrate or squint.
  • Crow's feet — the fan of lines at the outer corners of the eyes that appear when you smile or laugh.

Additional areas — such as a "gummy" smile, a downturned mouth, neck bands, a dimpled chin, or a heavy jaw related to clenching — can also be addressed, but these are typically discussed once your clinician has assessed your face at rest and in motion.

Who is a good candidate?

Good candidates are generally healthy adults who are bothered by dynamic lines and who want a natural, refreshed look — not a frozen one. A thorough medical review at your consultation is important, because certain neuromuscular conditions, active skin infections in the treatment area, pregnancy, and breastfeeding are reasons to postpone or avoid treatment. Your medications, allergies, and prior cosmetic history all factor into the plan.

At MedexClinic, consultations are led by Dr. Asim Nouman, MD, an experienced physician with 18+ years of clinical practice. A physician-led assessment matters here because anatomy varies — the same line in two different faces may need a very different approach.

What happens at your first visit?

Your first visit is designed to be unhurried. Here's the typical flow:

  • Medical intake — health history, medications, allergies, and cosmetic goals.
  • Facial assessment — your clinician watches how your muscles move at rest and during expression, then maps the areas that will and won't be treated.
  • Photos — standardized photos are taken so progress can be reviewed objectively at your follow-up.
  • Plan and consent — the recommended areas, realistic expectations, aftercare, and possible side effects are all reviewed before you agree to proceed.
  • Treatment — the injections themselves generally take only a few minutes once the plan is set. A very fine needle is used, and most patients describe the sensation as a quick pinch.

What does injection day actually feel like?

Most first-timers are surprised by how quick and low-drama the appointment is. The skin is cleansed, injection points are marked, and a series of small injections are placed into the targeted muscles. Numbing cream or a cool compress can be used if you're needle-sensitive, but many patients find they don't need either.

Immediately afterward you may see tiny raised bumps at each injection point — these settle within about 15–30 minutes. Mild pinkness is normal. You can usually return to your day; you'll just be asked to follow a short list of common-sense aftercare steps.

Aftercare: the first 24 hours

  • Stay upright for about four hours — no lying flat or face-down naps.
  • Skip strenuous exercise, saunas, and hot yoga for the rest of the day.
  • Avoid rubbing, massaging, or pressing on the treated areas.
  • Delay facials, microdermabrasion, and other face-down spa treatments for a couple of weeks.
  • Mild bruising can happen; arnica and cool compresses can help.

Your clinician will give you a written aftercare sheet tailored to the areas you had treated.

When will you see results — and how long do they last?

Wrinkle-relaxing injectables don't work instantly. Here's the general timeline most patients experience:

  • Days 1–3: No visible change yet — this is normal.
  • Days 3–5: Muscles start to feel less "grippy" during expressions; lines begin to soften.
  • Days 10–14: Full effect. This is the ideal time for a follow-up review, and for any small touch-ups if needed.
  • Months 3–4: Movement gradually returns as the effect wears off. Many patients schedule maintenance around this window.

Longevity varies with the area treated, muscle strength, metabolism, exercise habits, and how consistently you've been treated over time. Your clinician will help you plan a realistic maintenance rhythm rather than chasing a fixed calendar.

Are there risks or side effects?

Wrinkle-relaxing injectables have a long track record when placed by a trained clinician who understands facial anatomy. Common, temporary side effects can include:

  • Small bruises or pinpoint bleeding at injection sites
  • Mild headache in the first day or two
  • Temporary tenderness or a heavy-feeling brow while the product settles

Less common issues — such as a brow or eyelid that sits lower than intended — are usually temporary and are one of the reasons an experienced injector, a conservative starting plan, and a two-week follow-up matter so much. This is a treatment where who injects you is at least as important as what is injected.

How to get the most natural-looking result

  • Start conservatively. You can always add more at your two-week check; you can't remove what's already placed.
  • Treat the whole upper face as a unit. Softening only one area can throw off balance — a good injector plans for how muscles work together.
  • Keep a consistent schedule. Regular, moderate maintenance tends to look better than occasional, heavy treatments.
  • Pair with good skincare. Daily sunscreen, retinoids (as tolerated), and hydration protect the results you've paid for.
  • Ask questions. A confident clinician welcomes them.

Questions to ask before you book

  • Who will actually perform the injections, and what is their training?
  • Will a physician be involved in my assessment and plan?
  • What is your approach if I'm not happy at the two-week mark?
  • How do you decide how much product to use for a first-timer?
  • What is your aftercare and follow-up process?
