A doctor-supervised ketogenic diet can be a powerful tool for weight loss, blood-sugar control, and appetite regulation — but it isn't for everyone. Because keto shifts your metabolism into ketosis by dramatically reducing carbohydrates, it changes how your body handles fluids, electrolytes, and certain medications. That is exactly why a clinician should evaluate whether keto is a good fit for your physiology, your medications, and your goals before you start.

Who should not start keto without close medical oversight?

Keto is not appropriate for everyone. Several groups should either avoid it entirely or only attempt it under close physician supervision:

Why electrolyte monitoring matters on keto

When carbohydrates drop sharply, insulin levels fall, and the kidneys begin releasing more water and sodium. That's why the first week of keto can bring the classic "keto flu" — headaches, fatigue, muscle cramps, brain fog, lightheadedness, and constipation. Most of these symptoms are electrolyte-driven, not carbohydrate-driven.

A doctor-supervised plan focuses on three main electrolytes:

  • Sodium — often intentionally increased with broth, salt on food, and electrolyte drinks that don't contain added sugar.
  • Potassium — supported through leafy greens, avocado, salmon, and other whole-food sources.
  • Magnesium — commonly under-consumed; supplementation is sometimes recommended after review.

Hydration is equally important. Because ketosis is naturally diuretic, most patients need noticeably more fluid than they did on a standard diet. Your clinician can advise on targets that are appropriate for your kidney function, blood pressure, and activity level.

What does a sample first week on keto look like?

Every plan is individualized, but here is a realistic structure many patients follow in week one. The emphasis is on whole foods, adequate protein, generous non-starchy vegetables, and healthy fats — not on chasing the deepest possible ketosis.

Breakfast options:

  • Scrambled eggs with spinach, tomatoes, and avocado.
  • Greek yogurt (unsweetened, full-fat) with chia seeds and a few berries.
  • Smoked turkey and cheese roll-ups with cucumber slices.

Lunch options:

  • Grilled chicken over mixed greens with olive oil, lemon juice, olives, and feta.
  • Tuna or salmon salad on cucumber rounds with a side of roasted cauliflower.
  • Beef and vegetable stir-fry cooked in olive oil with a splash of low-sodium broth.

Dinner options:

  • Baked salmon with roasted asparagus and a lemon-tahini drizzle.
  • Lamb or beef meatballs simmered in tomato sauce over zucchini noodles.
  • Roasted turkey thighs with brussels sprouts and garlic butter.

Snacks: hard-boiled eggs, olives, nuts (in moderation), cheese, celery with almond butter, or plain Greek yogurt.

Fluids: water, unsweetened tea, black coffee, and clear broth for sodium. Skip sweetened drinks and fruit juices.

How does keto fit into a broader weight-loss plan?

Keto is one tool — not a lifelong prescription.
