How does a cholesterol doctor read a lipid panel?
Most patients see four numbers on a standard lipid panel: total cholesterol, LDL ("bad" cholesterol), HDL ("good" cholesterol), and triglycerides. Those matter — but they're the beginning of the story, not the end. A thoughtful review also considers the LDL-to-HDL ratio, non-HDL cholesterol, and whether triglycerides are drifting upward alongside blood sugar or waist circumference.
When the picture is unclear, we may order advanced testing: apolipoprotein B (apoB), lipoprotein(a) — often written Lp(a) — and high-sensitivity C-reactive protein (hs-CRP) for inflammation. ApoB in particular is one of the most reliable markers of the actual number of atherogenic particles circulating in your bloodstream, and it sometimes tells a very different story than LDL alone.
What your lipid panel actually shows
- LDL cholesterol — the particle most closely linked to plaque buildup in arteries.
- HDL cholesterol — helps clear cholesterol from the bloodstream; higher is generally protective.
- Triglycerides — rise with excess sugar, refined carbs, and insulin resistance.
- Non-HDL cholesterol — total minus HDL; captures every "bad" particle in one figure.
- ApoB — a direct count of atherogenic particles, useful when LDL looks fine but risk feels high.
- Lp(a) — a largely genetic marker; a single lifetime test can change your whole risk profile.
Statins vs. newer lipid-lowering options: how the decision is made
Statins remain the foundation of cholesterol treatment for most adults at meaningful cardiovascular risk. They are well-studied across decades, generally well tolerated, and available in a range of strengths that can be tailored to the individual. For many patients, a statin is enough — combined with lifestyle changes — to bring LDL and apoB into a healthy range.
But statins are not the whole toolbox. When LDL stays stubbornly high on maximum tolerated statin therapy, when a patient has a strong genetic driver like elevated Lp(a) or familial hypercholesterolemia, or when statins simply aren't tolerated, other classes come into the conversation. These include non-statin oral lipid-lowering options and injectable PCSK9 inhibitors — a newer class that helps the liver clear LDL from the bloodstream more efficiently.
The right combination depends on your risk category, your response to therapy, side effects, and preferences. That's a conversation, not a formula — and it's the kind of conversation a physician-led primary care practice is built for.
When family history changes the whole picture
If a parent, sibling, or grandparent had a heart attack, stroke, or bypass surgery before roughly age 55 (men) or 65 (women), your cholesterol numbers need to be read through a different lens. The same LDL that would be "borderline" for someone with no family history may be a red flag for you.
A few situations meaningfully change the plan:
- Familial hypercholesterolemia (FH) — an inherited condition where LDL is elevated from birth. Standard lifestyle changes rarely bring it into range alone.
- Elevated lipoprotein(a) — genetically determined, unaffected by diet, and often missed. It roughly doubles the value of testing for it once in a lifetime.
- Early-onset cardiovascular disease in first-degree relatives — a nudge toward earlier imaging (like a coronary artery calcium score) and more assertive LDL targets.
- Diabetes or metabolic syndrome in the family — often clusters with high triglycerides and low HDL, and shifts treatment toward metabolic health as well as lipids.
What role does diet actually play?
Diet matters — but not always in the way patients expect. For LDL cholesterol, the biggest levers are saturated fat and refined carbohydrates. For triglycerides, added sugar, sugary drinks, and excess refined starch are usually the dominant drivers. HDL responds most reliably to physical activity, weight loss, and quitting smoking, not to any specific food.
Practical, evidence-based patterns we discuss with patients include:
- Building meals around fish, skinless poultry, eggs, beans, lentils, and tofu instead of fatty processed meats.
- Cooking with olive oil, avocado oil, and nuts instead of butter and tropical oils.
- Choosing oats, barley, beans, and psyllium — soluble fiber sources that meaningfully lower LDL.
- Filling half the plate with vegetables and fruit, especially leafy greens and berries.
- Deglazing pans with low-sodium broth, lemon juice, or balsamic vinegar instead of other cooking liquids.
- Cutting back on sugary drinks and ultra-processed snacks — often the fastest way to move triglycerides.
No single food fixes cholesterol. A pattern eaten consistently for months does.

We review your full lipid history, your family history in detail, your other risk factors (blood pressure, blood sugar, weight, sleep, stress, activity), and your goals. If your labs are outdated or incomplete, we order updated testing — potentially including apoB and Lp(a).
From there we build a plan that may include lifestyle change, structured follow-up, additional testing (like a coronary artery calcium score in the right patient), and — if appropriate — medication.
